I have the Monday morning slot so, although my appointments do not start until 9am, I always arrive in the surgery at least half an hour before. My first job is to switch on my computer, check the post and then wander down to the front desk to have a chat to the reception staff. We exchange news of the weekend but the phones are already ringing and patients are arriving so any banter is short and succinct. My first appointment is at 0850 but by 9 o'clock I would have already seen at least two patients. The reason for this is that morning patients are predominantly elderly and tend to arrive in good time for their appointments. If they are early then I am pleased to see them. Effective time keeping is fundamental for a well-run surgery and there will inevitably be extra patients added to my list by the end of the morning.These extra patients may need an urgent ECG, or be asthmatic patients requiring emergency nebulisers. This is treatment room work, which requires speed and decisiveness. So the morning will be taken up with wound dressings, suture removal, ear syringing and injections. But in between appointments I may be testing urines and doing urgent blood tests. A five-minute coffee break at the desk will keep me going until my last patient who is generally seen at midday. I then use the next 30 minutes to clear up before I leave at 12 30. I usually go home to put my feet up because, this being Monday, means that I am on a split shift and have to return to the surgery at 3pm. The atmosphere in the surgery in the afternoon is very different from the morning. The frantic pace has slowed down. The phones are still ringing but the morning urgency seems to have settled. For me this is my favourite time. There is a more leisurely mood at work and because most of the appointments are longer (15-20 minutes) I have more time to talk to the patients. The afternoon work is predominantly chronic disease management. This is the time that we attempt to get the Quality and Outcome Framework (QOF) points. QOF is the programme that financially rewards GP practices for managing some of the common chronic diseases. So afternoon appointments are generally given to reviewing patients with asthma, diabetes or hypertension. This is also the time that we do travel clinic, cytology screening and childhood immunisations. We offer appointments until 6.30pm so that patients who require treatment at the end of their working day can be seen. I would like to offer even later appointments and feel that opening the surgery at later times would be beneficial. However this is not a popular proposal to some of the staff and so is not discussed. New proposals may change this because there are government plans to extend surgery opening hours.I often reflect on whether public attitudes to health have changed in recent years. For some patients, yes. They see that their lifestyle choices can be contributing to their chronic disease and they make concerted efforts to lose weight, stop smoking or increase exercise levels. Others, however, will deny that any of their daily living habits are responsible for their condition. I still have patients with asthma who, with astounding obstinacy, deny that their smoking habit has any effect on their ability to breath effectively. I never miss an opportunity for health promotion and hope that the patient heeds the advice and acts on it.If they don’t then that is their decision, but I want it to be an informed decision. So is practice nursing a good job? Unquestionably yes. The work is diverse, challenging and often demanding. I work in close-knit supportive team. There are four GPs and the practice has a strong teamwork ethic and a common vision. I like my shift patterns, although some nurses are surprised that we are expected to do split shifts. However the GPs feel that these shifts are needed to cover the surgery and they are my employers. In primary care it is still the case that ‘he who pays the piper plays the tune’. What of the future of practice nursing? Many practice nurses are now prescribers, they can be the surgery specialists in chronic disease management and they have a diverse range of skills. New government initiatives are introduced regularly, such as targets to reduce obesity and smoking. Next autumn [2008] will be the start of the cervical cancer vaccine for 12-year-old girls. Will the job of immunising these young women be given by the practice nurse? This has not been decided yet. What is significant is that practice nursing is evolving continuously and constantly challenging and that, I think, is why I enjoy it so much.

My First Time at the English Corner

It was a beautiful Sunday. Our teacher told us that there would be an English Corner near the gate of the People’s Park. I started off soon after breakfast.
