它们的意思如下：
given name等同于first name是名，教名。如字面意思，父母或者教父给起的名字。
middle name是名和姓之间的名字，是父母或亲戚所取，一般取长者的名或姓。

举例说明：
George Walker Bush名字的结构式：

译文：教名、首名+中间名+姓氏=全名
中间名字可以缩写：George W. Bush(美国习惯)
在英国教名也可以缩写：G. W. Bush
另外，很多时候，中间名可以略去不写：George Bush.
法律对名字的定义：
教名，出生和受教会洗礼时取的名字，是受法律承认的名字。
中间名，通常用缩写表示，由钟爱孩子的父母或亲戚所取，他们甚至把自己的名字直接取给孩子。法院不承认中间名是法定姓名的一部分。
姓氏，是有家族世代相传的。美国法律规定，妇女婚后要使用丈夫的姓，即使离婚，也应予保留，非经法律判决，不可恢复未婚时的姓。
西方姓名的历史：
在西方文明当中，姓氏（family names，家族名）这个概念出现的是较晚的。
比如英国，该国公认的姓氏起源是公元1086年，因为征服者威廉发布《末日审判书》，调查国家的人口分布，需要受调查者的介绍，所以拥有封地的贵族们才第一次采用姓氏。
