主诉尽可能用病人自己描述的症状，不用诊断用语。对当前无症状，诊断资料和入院目的又十分明确的患者可适当用诊断数语。应尽量简洁，一般不超过20个字。
“主诉”是住院病历中十分重要的内容之一，好的主诉不仅能反映病人本次住院最主要的病症或需求，还可正确决定现病史、既往史的书写布局和入院诊断的排序。
