请假是用来向老师或者单位领导等上级传达某人的请假信息。其格式见上表格。日期部分一般包括年、月、日，大多数情况下“年”可省去，因为请假条的内容多半是当日或近日的事情。称呼通常以Dear...开头，要顶格书写。正文内容要简短扼要，用语比较通俗，接近口语化。写请假条时，一定要把请假的理由和时间长短写清楚，以便于上级领导作决定或安排工作。
